Our personal and home support services are designed for individuals who want to remain in the comfort of their own home — whether due to age, illness, disability, or recovery — while receiving the care they need to do so safely.

The Access Care Foundation Mobile Health Unit delivers essential health services directly into the community — helping reduce barriers and improve access to care.
For many individuals, transportation, mobility challenges, cost, and isolation create barriers to essential care. Our mobile clinic helps remove those barriers by bringing care directly to neighbourhoods, community centres, shelters, and gathering places where it's needed most.
Our team of healthcare professionals delivers preventive, responsive, and dignified care to populations who have historically been underserved by traditional models of healthcare delivery.
