01 — In the Home Personal & Home Support

Living safely & with dignity, at home.

We support individuals to live safely and with dignity at home through compassionate home support services — including personal care, companionship, and respite.

Home is more than a place. It's where memories are made, routines are kept, and identity is preserved. Our home support program is designed for individuals who want to remain in the comfort of their own home — whether due to age, illness, disability, or recovery — while receiving the care they need to do so safely.

Our trained support workers offer a holistic approach to care, blending practical help with emotional connection. Every visit is grounded in respect for the person, their independence, and their preferences.

03 — In the Community Mobile Health Unit

Bringing care directly to the community.

The Access Care Foundation Mobile Health Unit delivers essential health services directly into the community — helping reduce barriers and improve access to care.

For too many individuals, transportation, mobility, cost, and isolation stand between them and the care they need. Our mobile clinic exists to remove those barriers — driving care to neighbourhoods, community centres, shelters, and gathering places where it's needed most.

Our team of healthcare professionals delivers preventive, responsive, and dignified care to populations who have historically been underserved by traditional models of healthcare delivery.

05 — Practical Help Essential Supplies & Donation Support

Filling the gaps that keep people safe.

Where needs cannot be met through existing services alone, the Foundation also helps provide access to essential supplies through community donations and Foundation-funded supports.

Sometimes care comes down to the simplest things — a walker that prevents a fall, a comfort blanket during recovery, an incontinence supply that preserves dignity, or a piece of safety equipment that lets someone stay at home a little longer.

Through partnerships with donors, community members, and charitable organizations, we work to ensure that no one goes without the essentials they need to remain safe, supported, and dignified at home and in their community.
